*POSITION SUMMARY*
An outpatient physical therapy clinic located in Greenville, TX with award winning therapists that are known for quality care, individualized treatment plans, appropriate patient progression, high patient satisfaction, and great relationships with our referring providers. We specialize in orthopedic, industrial, and sports medicine rehabilitative services. We are currently experiencing exciting new growth, as we are adding new orthopedists to our practice, and will soon move into a new 3900 sf PT facility!.
*POSITION SUPERVISORY RESPONSIBILITIES*
* *Reports To: *Sports Medicine Manager
* *Supervises: *NONE
*POSITION REQUIREMENTS*
* *Minimum Education: *Graduate of an approved physical therapist program with evidence of current PT licensure in the state of Texas. Current CPR certification.
* *Minimum Work Experience*: None
* *Required Licenses/Certifications: *Evidence of current licensure in the state of Texas. CPR certified
* *Required Skills, Knowledge, and Abilities: *Effective verbal and written communication skills. Ability to meet minimum hiring standards.
*PHYSICAL REQUIREMENTS*
* Able to lift up to 50 pounds without assistance using proper technique
* Frequent walking, standing, bending, and reaching throughout the workday
* Sufficient visual and auditory acuity for clinical and documentation duties
* Fine motor skills for equipment handling, documentation, and communication tasks

*JOB SPECIFIC FUNCTIONS*
1. Demonstrates an understanding of and adherence to the HMHD Compliance Plan.
2. Conduct reflects HMHD’s values and a commitment to HMHD’s Code of Conduct.
3. Attends the required corporate integrity and compliance training and education programs.
4. Demonstrates proficiency in understanding the materials presented during the corporate integrity and compliance training and education program.
5. Complies with all HIPAA standards.
6. Evaluate and treat patients in accordance with the Texas Board of Physical Therapy Examiners
7. Develop and implement individualized plans of care, modifying as appropriate
8. Establish and track short-term and long-term goals
9. Verify physician orders and seek medical advice as necessary
10. Complete daily documentation, progress notes, and discharges in a timely manner
11. Educate patients and families on care plans and track their responses
12. Participate in discharge planning and interdisciplinary care
13. Supervise and direct assistants and aides
14. Provide clear instruction and oversight for delegated care
15. Adhere to compliance standards, including HIPAA and corporate integrity training
16.Attend staff meetings, trainings, and professional development sessions
17.Support performance improvement and professional education activities
